Required Sports Physicals
If your children will participate in school athletics (clubs or sports), they will need a complete six-page
PIAA CIPPE physical packet. (There are some places where students must sign and that is true of even the young students.) Also pay attention to when dates are needed.
The May 1 or later doctor-signed physical will be valid all year long. However, for winter and spring sports, you will also need to submit a simple, parent-signed form that confirms that your child's health has not changed since he or she received doctor's approval. This is the
CIPPE: Section 7: RE-CERTIFICATION BY PARENT/GUARDIAN.
Should your child be injured during the school year and removed from athletics, when the doctor re-clears your child for athletics, the doctor will need to sign
CIPPE: Section 8: Re-CERTIFICATION BY LICENSED PHYSICIAN OF MEDICINE OR OSTEOPATHIC MEDICINE. This is the doctor confirming that your child's health has returned to a cleared status. If your child is injured, please email the Athletic Director and the coach and they will let you know if you need this form. If the doctor tells you to return to the doctor before being cleared to go back to gym class or athletics, you definitely need this form. If your child suffers a concussion, you definitely need this form. If your child has a minor injury that the doctor tells you to self-monitor and return when feeling able, you may not need this form. When seeing a doctor for an injury, the Athletic Director's advice is to bring this recertification form to all appointments in case the doctor is ready to clear your child to return to play.
You will
register your child to play sports using our electronic registration. This is where you will submit the physicals and other required forms, list allergies or other medical information, as well as emergency contact info, before being approved to play. Do not turn in your physical to the office. You will upload them when you register.
